Tatalaksana Depresi Pasca Stroke

Abstract


Stroke merupakan penyebab kematian dan kecacatan terbesar kedua di dunia. Depresi pasca stroke (PSD) merupakan komplikasi pasca stroke yang paling sering terjadi dan memiliki luaran yang buruk. Diperlukan pengobatan yang adekuat untuk mencapai remisi pasien. Tujuan: Untuk mengetahui manajemen gejala depresi pada pasien depresi pasca stroke. Metode: Database elektronik yang digunakan: Pubmed, Cochrane, dan SpringerLink serta artikel yang diidentifikasi dari tahun 2016-2022. Artikel yang memenuhi kriteria inklusi dan eksklusi akan diperiksa. Hasil: Tatalaksana depresi pasca stroke meliputi tatalaksana farmakoterapi yang dapat dipertimbangkan meliputi Venlavaxine,  Escitalopram dan Citalopram. Manajemen nonfarmakoterapi yang dapat dipertimbangkan sebagai adjuvan yaitu kombinasi elektroakupunktur dan intervensi psikologis, Behavioural Activation Therapy, dan mindfulness-based intervention. Kesimpulan: Antidepresan golongan SSRI dan SNRI dapat digunakan pada pasien  PSD. Escitalopram dapat memperbaiki afek depresi namun tidak memperbaiki gejala kognitif dan neurovegetatif. Citalopram efektif digunakan pada pasien PSD dengan onset dini. Venlafaxine memiliki efektifitas lebih baik dan minimal terjadi efek hiponatremia dibandingkan Fluoxetin  Manajemen-manajemen nonfarmakoterapi dapat dipertimbangkan sebagai adjuvan PSD yaitu kombinasi elektroakupuntur dan psycological intervention, CBT, CCT, Behavior Activated therapy, serta Mindfulness-based intervention.
